Accordant, a CVS Health affiliate, made an announcement this week revealing that it has been awarded Case Management Accreditation from the National Committee for Quality Assurance (NCQA).

Accordant received the accreditation based on its commitment to improving health outcomes and lowering costs through high quality service and care oriented around the patient.

Accordant provides its services and individualized support to members with rare chronic diseases and other conditions on behalf of health plans, employers and third-party administrators. Their goal is to help patients circumvent avoidable medical costs while providing a better overall health experience and outcome.

Through its case management, Accordant handles care transitions and detects and averts potential difficulties to help members avoid expensive emergency room visits and extended stays in hospitals. The program operates under published guidelines and consensus statements and also adheres to standards of medical practice.

The NCQA’s evaluation of health care companies involves 10 demanding standards of case management. These standards were created by experts in the field, which include researchers and regulators from both the state and federal levels.

NCQA is a nonprofit organization that accredits a variety of health care organizations, with the aim of improving the quality of health care.
