naviHealth Inc. to acquire Curaspan Health Group Inc.
naviHealth is a Cardinal Health company that focuses on post-acute care management. Curaspan provides an electronic platform that facilitates patient transitions from hospitals to post-discharge providers.

Health care’s evolving reimbursement environment has accelerated the need for clinicians to seek effective tools for managing patients in a value-based care world, and we believe that the combination of our existing clinical decision support tools and clinical analytics with Curaspan’s leading care transitions platform and post-acute network will create a compelling customer offering.”
The addition of Curaspan, which is installed in more than 600 hospitals, will expand naviHealth's capabilities in the post-acute market. The ability to communicate patient information and referrals to post-discharge facilities, such as nursing homes, acute care hospitals, dialysis centers and other providers will improve the patient experience.
