The American Society of Health-System Pharmacists (ASHP) recently published “Extemporaneous Formulations for Pediatric, Geriatric and Special Needs Patients” to aid in the treatment of patients who require medication dosages and forms that are not available commercially.

The new publication is the third edition released by the ASHP.  This latest addition of the “Extemporaneous Formulations for Pediatric, Geriatric and Special Needs Patients” contains formulations for 197 medicines that are based on evidence. It also includes 39 new and two updated monographs.

The ASHP resource also contains thorough instructions for the preparation of a wide range of treatments that include elixirs, solutions, suspensions and syrups. Each of the new monographs were constructed after extensive literature review and also contain ingredients and suitable substitutes, guidance on proper conditions for storage, recommended expiration dates and appropriate references.

The publication also includes the ASHP’s technical assistance bulletin for compounding nonsterile products, a USP chapter summary, a summary of the requirements for state compounding and the standardized concentrations list that was established by the Michigan Pediatric Safety Collaboration.
