The CVS Health Foundation has released new data from programs that were part of a collaborative effort with the National Association of Free & Charitable Clinics (NAFC) and the National Association of Community Health Centers (NACHC) as part of CVS Health’s multi-year, $5-million commitment to increase nationwide access to health care in communities.

The foundation has also announced new grants in the total of $1.5 million that will be distributed to 46 community health care centers and free clinics this year.

The donations are part of CVS Health’s ongoing effort to provide access to better health care by increasing access to that care as well as the prevention and management of chronic diseases. By collaborating with the NAFC and NACHC, the CVS Health Foundation is providing more access to coordinated health care that is greatly needed in communities nationwide.

The CVS Foundation announced 55 total grant recipients in April of last year that have made significant progress toward improving coordinated care, managing chronic conditions and increasing the access to care.
