CVS launches Transform Diabetes Care

CVS Health recently launched Transform Diabetes Care, a program dedicated to helping clients improve member health outcomes.
Transform Diabetes Care also aims to decrease costs with aggressive trend management and cut medical costs by redefining medication adherence. CVS notes that pharmacy benefits management clients who use the program will be able to save roughly $3,000 to $5,000 every year.
“The Transform Diabetes Care program focuses on three key components to help improve outcomes and reduce overall health care spend in diabetes: medication adherence, A1C control and lifestyle management," CVS Health Chief Medical Officer Dr. Troyen Brennan said in a statement.
Brennan notes that the program can use resources from across the company. With CVS’ large footprint, the program will have access to personalized support through many touch points.
“What makes our face-to-face counseling and education at our more than 9,600 pharmacy locations and more than 1,100 MinuteClinic locations so effective is that it happens when patients are already thinking about their health,” Brennan said.
